Dix-Hallpike Maneuver
- Dix Hallpike is used to confirm a diagnosis of BPPV in either the anterior or posterior canals.
- During the test, the patient's eyes will exhibit nystagmus with a torsional component.
- Posterior canal is ruled in if there is upbeating nystagmus, and the direction of torsion demonstrates which side is affected.
- Anterior canal is ruled in if there is downbeating nystagmus, and the direction of torsion demonstrates which side is affected.
- Canalolisthiasis is ruled in if the nystagmus lasts less than 60 seconds.
- Cupulisthiasis is ruled in if the nystagmus lasts more than 60 seconds.
- If downbeating nystagmus occurs without a torsional component , a central lesion is expected.
